Services may be provided in this health care facility by the facility as well as by other health care providers who may separately bill the patient and who may or may not participate with the same health insurers or health maintenance organizations as the facility.
Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider. Upon being scheduled for surgery we will contact your insurance carrier for verification of your insurance. After identifying the extent of your coverage we can determine an estimate of your financial responsibility if any. You will be contacted 24-48 hours in advance of your scheduled procedure regarding your financial estimate, payment is required the date of your procedure. For your convenience, we accept Master card, Visa, Discover, American Express, Care Credit, personal check, and cash. If you need to make payment arrangements please consult with the billing staff in advance.
In addition to your bill from the center, you may receive a bill from other entities. Click here for a list of possible service providers that may charge you.
As a service to you we will bill your insurance company. However, you will be liable for those costs which your insurance company does not cover.
